We are currently seeking a professional that possesses strong leadership and interpersonal skills for our to join our exceptional and welcoming centralized admissions team. The LPN Chart Review is responsible for using clinical knowledge and to assess patients for admission to post-acute and long-term care, as well as outreach to our referring partners.
In this position you will:
Review medical charts of referred patients to determine appropriateness for admission
Work with our hospital partners to facilitate safe and effective transfers, develop relationships and grow admission and census levels.
Promote quality and cost-effective outcomes and a seamless process for post-acute admissions by communicating with nurses, hospital case managers and all available resources.
Following up on outstanding items needed to close medical records
Corresponding with various departments to ensure efficient transitions of care
Other responsibilities may include: Answering inquiries regarding admission in a timely, enthusiastic and positive manner, Verifying insurance coverage, completing internal documentation and preparing medical records for admission
Position Details
Admissions Liaison is a full-time position in-office

Strongly Recommended Skills & Certifications:
Experience with admission screening or case management or strong knowledge of medical conditions and terminology
Experience with Point Click Care
Strong clinical assessment skills through review of medical records
Excellent communication and interpersonal skills
Experience in care transitions in a hospital or skilled nursing; Post-Acute experience preferred.
